How do I file a claim under Dial Direct®?
To file a claim, simply visit www.mywirelessclaim.com or by calling1-888-352-9183. Please be prepared to provide the following information to complete your claim:
* Your wireless phone number
* The make and model of your phone
* The electronic serial number (ESN) of the phone you are claiming
* What happened to your phone (date, time and location of the incident)
* A police report number will be required for a stolen phone
Note: A Proof of Loss form may be required to complete your claim.
